I wondered that myself, but in the video you can hear Wilson and other house Republicans causing a stir a good 10 seconds before the "You lie". A cameraman taking a burst shot at the general direction of their reactions could have caught all of that in a moment. The cameraman in question is Chip Somodevilla, a news photographer for GettyImages.
